In order to provide the most beneficial health and wellness programs for your company, Healthy Ventures, provides each employee with the opportunity to complete a written or on-line health assessment survey. This survey consists of basic health related questions that allow Healthy Ventures to determine the needs and wants of each individual employee as related to his or her health. Along with this survey, a health risk appraisal is also taken for each employee. This appraisal consists of (i) height and weight measurements, (ii) waist, (iii) blood pressure readings, (iv) glucose levels, and (v) the results a of cholesterol profile.
